The short version

You’re handing over information about your child. You should know exactly where it goes.

This page describes what this website collects, who processes it, and how to have it removed. It’s written in plain language on purpose.

Everything below applies to planetsensorytherapy.com. Anything you share with the clinic in person, on the phone, or during care is handled by the clinic directly, not by this website.

What this site collects

There are three forms on this site, and they’re the only places the site asks you for anything.

The appointment request. Your name, phone number, email address, and what you wrote about what you’re noticing in your child. It also records which page you were on when you sent it.
The benefits check. First and last name, date of birth, ZIP code, insurance provider, member ID, group number, phone and email. This is the information an insurer needs before anyone can tell you what your out-of-network benefits actually cover.
The 30-second sensory check. Three multiple-choice answers. It doesn’t ask for your name, your email, or your child’s name, so those answers arrive with nothing identifying attached. If you go on to request an appointment in the same visit, those three answers are carried into that request so we have the context.

The information you send is used to answer you and to arrange care. That’s the whole purpose.

Who handles a form after you send it

This website is hosted on Netlify, and Netlify processes every form submission before it reaches the clinic. That means your submission passes through their systems, and their handling of it is governed by their own terms.

Submissions may also be passed into the clinic’s customer relationship management system, which is the tool the clinic uses to keep track of who reached out and to make sure someone actually replies to you.

Text messages

The appointment request form has a checkbox for text messages. Ticking it is your consent to receive text messages from Planet Sensory about your appointment.

Message and data rates may apply, depending on your carrier and your plan.

Reply STOP to any message to stop receiving them. You can also call or email and ask us to turn them off.

If you leave the box unticked, you won’t be texted through that consent.

Cookies and things loaded from elsewhere

Two parts of this site are loaded from third parties, and both can set their own cookies in your browser once they load.

A Google Reviews widget, which pulls the clinic’s reviews live from Google through a third-party embed.
A Google Maps embed, which shows the map to 826 Merrick Rd.

Those cookies are set by those companies, not by the clinic, and the clinic has no control over what they store. Your browser settings control whether third-party cookies are allowed at all.

Having your information deleted

Ask, and it’s removed. You don’t have to explain why.

Email ester@planetsensorytherapy.com or call (516) 744-7550 and say you want the information you submitted through the website deleted.

You can use the same two contacts to ask what was submitted, or to correct something you got wrong when you typed it.

Children

This site is written for parents and guardians, and the forms are meant to be filled out by an adult. Please don’t have your child submit them.

Changes to this page

If what the site collects changes, this page changes with it, and the date below changes too.
