You’ve been thinking about this a lot longer than this form will take. It asks what you’re noticing about your kid, not for a diagnosis you don’t have yet.

Once you send it, it comes straight to the clinic and a real person reads it. We follow up during business hours to talk it through and find a time that actually works around school.

You don’t need a referral to start. In New York, pediatric occupational therapists can evaluate and treat without a physician referral for up to 30 days. That applies to non-restorative services such as handwriting, learning and developmental skill training, when your child is not in acute care.

The first visit is a comprehensive initial evaluation, one-on-one with a licensed occupational and physical therapist. You watch the whole thing live on the waiting-room screen, and you hear what we found before you leave.
